FAQ: Coverage Changes

"I need to adjust the payroll for an employee on my policy. How do I go about doing this?"

The first step is to call your agent. Your agent will need to complete an ACORD Form 175 FL (2000/11) Monthly Change Sheet. You will need to provide your agent with a copy of the latest UTC 6 form or any other payroll documentation showing this adjustment.

"I have moved my business location. Do I need to notify my workers’ compensation carrier and if so, how do I go about doing this?"

Yes. You should immediately notify your agent of this change. Please fax 888-871-7474, Attn: Policy Services, or mail notification on your company letterhead to: FUBA Workers’ Comp, PO Box 1303, Tallahassee, FL 32302, Attention: Policy Services.
