Claims

Employers insured with FUBA Workers' Comp are provided with claims and medical management services through United Self Insured Services (USIS), located in Orlando. This program offers comprehensive services which include claims management, medical care management and a preferred network of providers.

Florida law requires insureds to report all workplace injuries to us within ten days of the injury.  When a work-related accident/injury occurs, follow these steps:

Emergency Situation

Step 1
Call 911 or seek medical help immediately.

Step 2
Promptly notify USIS at 1-800-444-9098 within 24 hours following the initial emergency treatment so that a nurse case manager can further coordinate the injured workers' medical care.

Non-Emergency Situation

Step 1    
Promptly notify USIS at 1-800-444-9098 to report any injury.

Step 2
After reporting the injury, request to speak with the nurse case manager to obtain assistance in identifying and locating the nearest doctor within the provider network.

Step 3
With guidance from the nurse case manager, make arrangements for your injured employee to see a doctor within the network.
