Become An Agent

If you are a licensed agency currently operating in the state of Florida and are interested in becoming an approved agency with FUBA Workers' Comp, please visit the Forms Library to obtain the necessary documents required in order to be considered for approval.

Applications for Agency Approval are reviewed by a committee comprised of Management and the Marketing/Sales Department.  An agency’s location, current book of workers’ compensation business, and ability to meet production goals will be taken into consideration. 

FUBA Workers' Comp requires a production commitment, tailored to each agency, and we will monitor your progress throughout the year.

We require our agencies to join Florida United Businesses Association (FUBA), the sponsoring trade association of FCBI.  FUBA is a not-for-profit organization that represents and protects the interests of small business throughout the state of Florida.  Dues are $85 annually.

Be sure to include the following:

  • proof of $1 million of E&O coverage
  • copy of principals' and producers' 220 licenses

If your agency is approved, you will need to pay the $85 member dues to FUBA.  Once payment is received, you will receive a FUBA Workers' Comp Agent Kit containing Underwriting Guidelines, Payment Information, Commission Information, Claims/Safety Information, and FUBA Member Benefits.  A three digit code will be assigned to your agency at this time and will be used to identify all of your agency transactions and records with FUBA Workers' Comp.   Please use this code on all correspondence with us.
