Agency Services
The Agency Services Department is a division of the Marketing/Sales Department and designed to provide our valued agents with the tools and support they need to write with FUBA Workers' Comp.
Agency Services provides:
- Agency approval to write business with FUBA Workers' Comp
- Agency Kits - including Underwriting Guidelines, Payment Plans, and Member Benefits
- Forms - including the FCBI Supplemental Application and FUBA Membership Application
- Marketing materials to assist you in selling FUBA Workers' Comp
- Agent of Record processing. In order to request an Agent of Record change, please note the procedures below:
- All Agent of Record (AOR) requests must be made on either the appropriate ACORD AOR form or the insured's letterhead and signed by an owner or officer of the business. Faxes are acceptable.
- FUBA Workers’ Comp will notify the existing agency of the request. The existing agency has 10 working days to secure a rescinding letter. Please note: Although the ACORD AOR form has the option of waiving the 10 day rescinding period, FUBA Workers’ Comp will not waive the rescinding period.
- If a rescinding letter is received, FUBA Workers’ Comp will notify the agency requesting the change that no further action will be taken.
- If no rescinding letter is received within the 10-day period, FUBA Workers’ Comp will send an Endorsement to the new agency notifying them of the change.
- All AOR changes will not be made between the 1st and the 10th of the month following FUBA’s completion of the AOR procedure.
- FUBA Workers' Comp protects the commission of new businesses for the writing agency during the first twelve months of the policy’s inception.
- Approved agencies cannot request AOR changes until they meet the required new business premium production goal set for their agency.
- Any AOR requests during the month of March will be held and processed effective April 1st of the following Fund Year.
- The Agent of Record listed on the policy on the last day of the policy period (March 31st) is responsible for any commission adjustments related to final audit billing for the entire Fund year.
- AOR requests from cancelled agencies will not be processed.
- AOR requests on cancelled policies or policies pending cancellation will not be processed. It is the requesting agency’s responsibility to re-submit the request once the policy has been reinstated.
- FUBA Workers' Comp reserves the right to make any changes or exceptions to this policy at any time without notification.
If at any time you need help, whether it be explaining FUBA Workers' Comp to your clients or you need a supply of forms, please email or call the Agency Services Department at 888-262-4483.
